These Terms and Conditions ("Terms") apply to your use of the Life Line Community Wellness ("Life Line") website and the services offered through it. By using our website or contacting us, you agree to these Terms. If you do not agree, please do not use the website.
About our services
Life Line is a Bronx based community organization and a partner in the SOMOS Social Care Network. We help eligible Medicaid and Medicare members connect to meals, grocery support, transportation, care management, and preventive health services. Our website provides information about these programs and lets you check eligibility, ask questions, and request support.
Eligibility and enrollment
Checking your eligibility or submitting a form does not guarantee enrollment in any program. Final eligibility depends on your health plan, program rules, and a review by our team. We will let you know what you qualify for and what happens next.
Not medical advice
The information on this website is for general educational purposes and is not medical advice. It does not replace care from a licensed provider. Always talk to a qualified professional about your health. This website is not for medical emergencies. If your situation is life threatening, call 911.
Using our website
When you use our website, you agree that you will not:
- Use it in any way that breaks the law or harms others.
- Submit false, misleading, or someone else's information without their consent.
- Attempt to disrupt, damage, or gain unauthorized access to the website or its systems.
- Copy, resell, or misuse our content without permission.
Forms and referrals
When you submit a contact form, eligibility check, or referral, you confirm that the information is accurate to the best of your knowledge and that you have permission to share any information about another person. We use this information to respond and to coordinate services, as described in our Privacy Policy.
SMS and 10DLC text messaging
Life Line Community Wellness operates two separate text message programs: one for service messages and one for marketing messages. They are independent. You may join either one, both, or neither.
You opt in on our contact form by checking the clearly labeled checkbox for that program. Each program has its own checkbox, no box is checked for you in advance, and checking one does not enroll you in the other. Providing your mobile number and agreeing to receive texts is voluntary. It is not a condition of receiving services or of any purchase.
Program 1: service and appointment messages
If you opt in to this program, we may send you:
- Service updates about the programs you are enrolled in or asked about.
- Appointment and visit reminders.
- Eligibility and enrollment notifications, including requests for missing information.
- Care coordination messages about meals, grocery support, transportation, and referrals.
- Replies to questions you send us.
Message frequency varies, because these messages are sent in response to your enrollment, your appointments, and messages you send us.
Program 2: marketing and promotional messages
If you opt in to this program, we may send you:
- News about new, expanded, or seasonal programs and services.
- Health and nutrition tips and wellness education.
- Invitations to community events, outreach days, and workshops.
- Open enrollment, renewal, and re-certification reminders.
You will receive up to 4 marketing messages per month. We will not send you marketing texts unless you check the marketing consent box. Consent to receive marketing messages is not a condition of any purchase or of receiving services.

How to opt out and opt back in
You may opt out at any time by replying STOP to any text message from us. You will receive a final message confirming that you have been unsubscribed, and we will not send you further texts in that program. You may also opt out, or leave only one of the two programs, by contacting us at office@lifelinecw.com or (929) 696 1544.
If you change your mind, you can opt back in by replying START to resume messages, or by contacting us and asking to be re-enrolled.

Carrier liability
Mobile carriers are not liable for delayed or undelivered messages.
Privacy and consent
Your mobile information will not be shared with third parties or affiliates for their own marketing or promotional purposes. Text messaging originator opt-in data and consent are not shared with any third parties. We keep a record of your consent, including when and how you gave it, for as long as required. Our content
The text, graphics, logos, and other materials on this website belong to Life Line Community Wellness or our partners and are protected by law. You may view and share our public information for personal, non commercial use, but you may not reproduce it for other purposes without our written permission.

Our website may link to other organizations, such as health plans or community resources. We do not control those websites and are not responsible for their content, services, or practices.
Disclaimers
