Life Line Community Wellness ("Life Line," "we," "us," or "our") respects your privacy. This Privacy Policy explains what information we collect through our website and programs, how we use it, and the choices you have. By using our website or contacting us, you agree to the practices described here.
Who we are
Life Line is a Bronx based community organization and a partner in the SOMOS Social Care Network. We connect eligible Medicaid and Medicare members to meals, grocery support, transportation, care management, and preventive health services.
Information we collect
We collect information in the following ways:
Information you give us
- Your name and contact details, such as phone number, email address, and mailing address.
- Your preferred language and how you would like us to reach you.
- Details you share in a message, contact form, or eligibility check, including the services you are interested in and basic information about your household or health needs.
- Information a caregiver, family member, or provider submits on your behalf with your consent.
Information we collect automatically
- Basic technical information such as your device type, browser, and general location.
- How you use our website, so we can keep it working well and improve it over time.
How we use your information
- To check whether you may qualify for our programs and to enroll you if you choose to move forward.
- To respond to your questions and follow up with you in your preferred language.
- To coordinate meals, transportation, care management, and referrals on your behalf.
- To operate, maintain, and improve our website and services.
- To meet legal, regulatory, and program requirements.
How we share your information
We do not sell your personal information. We share it only as needed to provide services and as described below:
- With the SOMOS Social Care Network and health plans, so you can access the programs you qualify for.
- With trusted service partners who help us deliver meals, coordinate rides, or operate our website, and who are required to protect your information.
- When required by law, or to protect the safety, rights, or property of members, staff, or the public.
All the above categories exclude text messaging originator opt-in data and consent; this information will not be shared with any third parties.
Health related information
Some of the information you share may relate to your health or eligibility for Medicaid and Medicare programs. We treat this information as sensitive and limit access to staff and partners who need it to serve you. Where our partners are covered by the Health Insurance Portability and Accountability Act (HIPAA), that information is also handled under the protections that apply to them.
How long we keep your information
We keep your information for as long as needed to provide services, to follow up with you, and to meet legal and program record keeping requirements. When it is no longer needed, we take reasonable steps to delete or de identify it.
How we protect your information
We use reasonable administrative, technical, and physical safeguards to protect your information. No method of transmission or storage is completely secure, so we cannot promise absolute security. Please avoid sending sensitive details, such as full medical records, through the website contact form.
Text messaging (SMS)
We run two separate text message programs, and you choose each one independently on our contact form: service and appointment messages, and marketing and promotional messages. Checking one box does not enroll you in the other, and neither is required to receive services.
If you opt in, we collect your mobile number, which program or programs you agreed to, and the date and time of your consent. We keep that record so we can show that you asked to hear from us, and so we can honor an opt-out.
Your mobile information and text messaging opt-in consent are never sold, rented, or shared with third parties or affiliates for their own marketing or promotional purposes. We share your number only with the messaging providers who deliver texts on our behalf, and only so that they can deliver them.
You can stop messages at any time by replying STOP, or reply HELP for assistance. You may also leave one or both programs by contacting us at office@lifelinecw.com or (929) 696 1544. Your choices
- You can ask us what information we hold about you and request a correction.

Children's privacy
Our website is intended for adults seeking services for themselves or someone they care for. We do not knowingly collect information from children without the involvement of a parent or guardian.
